Essential Duties:

General:

\nAttend meetings as required by the Program Director, Laraway Youth & Family Services, and Local Educational Agency (LEA)\nAttend trainings to increase knowledge and skills and as required\nDevelop, maintain and modify services to assist clients and their families in gaining access to needed medical, social, educational, vocational, housing, financial, and other services\nMaintain contact with other professional service providers, LEA, LEA staff and others, for the purpose of case review or consultation regarding services to a specific client\nDevelop treatment plans to deliver, or provide for the delivery of, services identified by the treatment team\nWork cooperatively with schools, social rehabilitative services and other agencies providing services to clients and family members of clients.\nFill in as school-based interventionist for clients when available\nMaintain a positive teamwork approach with all other staff and consultants in providing services to the youth\n

Administrative:

\nCommunicate behavioral data to appropriate treatment team members\nProvide monthly case management and specialized Rehabilitation Summaries as required by Program Director\nProvide assistance in the creating and scheduling of Backpack Summer Program(s) and other program activities\nOther duties as assigned\n

Job Description
