POSITION SUMMARY
The Promise Senior Services Community Liaison is a frontline staff who works with individuals from diverse backgrounds and has a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison between social services and the community to facilitate access to resources and improve the quality and cultural competence of service delivery. They also build individual and community capacity by increasing knowledge and self-sufficiency through a range of culturally and linguistically appropriate activities and services such as outreach, community education, and advocacy. This position is designed to work with South Salt Lake (SSL) residents 55 years of age or older and connect them to resources that promote health, safety, and independence, while supporting them in navigating services, addressing barriers, and making connections. ____________________________________________________________________________________________________
ESSENTIAL RESPONSIBILITIES AND DUTIES 1.
Community OutreachEngagement 1.1. Builds relationships in the community with residents age 55+ and those who have connections with them. 1.2. Assesses needs and facilitates connections to services, programs, and resources for residents age 55+, including those who may be unserved or underserved. 1.3. Works with residents age 55+, senior housing and care facilities to creatively solve obstacles to accessing resources such as public assistance programs, health care, food security, education, basic technology/digital literacy, transportation, and employment. 1.4. Provides mental health resources and information at community events and activities. 1.5. Conducts home, office, and public location visits to assist residents age 55+ and their families, ensuring equitable access to outreach programming. 1.6. Assists residents age 55+ in applying for financial support and refers them to appropriate sources for additional services. 1.7. Monitors clients age 55+ for changes in housing, medical, emotional and other situations and provides follow-up assistance as necessary. 1.8. Promotes and attends community engagement events, such as Promise SSL (PSSL) quarterly family nights and citywide events. 1.9. Recruits residents age 55+ to participate in a variety of community councils, coalitions, and focus groups. 1.10. Identify ways to connect or build a social infrastructure to support residents age 55+ across the City. 2. Collaboration 2.1. Acts as a facilitator between residents age 55+ and community organizations. 2.2. Builds and maintains relationships with community partners, senior centers, senior living and care facilities, and community agencies. 2.3. Provides short-term counseling for residents age 55+ and acts as a liaison between various community service providers. 2.4. Interacts regularly with the PSSL Family and Community Engagement team for specific case management, to share concerns and to arrange appropriate additional services. 2.5. Participates in multiple cross-functional groups, committees, and boards to support residents age 55+ across the City. 2.6. Works with supervisors to identify and expand partnerships that serve residents age 55+ in the SSL community. 2.7. Works collaboratively with the PSSL Family and Community Engagement Team and partners to align efforts and streamline services. (Housing, Food, Digital Access, and others). 2.8. Recruits and trains volunteers to work with residents age 55+. 2.9. Recruits and trains residents age 55+ to work with PSSL youth programs. 3. Administration 3.1. Supports community center and participant needs, including: 3.1.1.1. Maintaining regular office hours at the SSL Co-Op; 3.1.1.2. Opening and closing, staffing the front desk, maintaining clean and organized spaces; 3.1.1.3. Supporting the overall safety, order, and accountability of participants and staff; and 3.1.1.4. Assisting participants in accessing programs, resources, classes, or services (printing, devices, etc.). 3.2. Maintains logs from home visits and other outreach activities through Unite US and other tracking tools. 3.3. Maintains confidential client files and inputs all data and interactions in the Unite US database and other tracking tools. 3.4. Takes notes and provides reports/updates from meetings. 3.5. Gathers resource materials for distribution at community events and sites. 3.6. Differentiates between basic client needs and those of a more serious nature. Can identify an emergency, call 911, and inform the Director. 3.7. Attends relevant seminars and training programs to maintain knowledge of community resources and service delivery systems. 4. Participate in the South Salt Lake Cares Coalition (Education, Health, Arts, Neighborhood, Jobs and Economy, Safety, Housing, Interfaith, and Social Impact) and supervise City events as assigned.
