Job Summary:

The Center at Arrowhead is seeking a dynamic, relationship-driven Admissions & Community Liaison to serve as the primary bridge between our facility and the community. The ideal candidate will be responsible for driving census growth by building strong partnerships with local hospitals, physicians, case managers, and social workers, while ensuring a seamless transition for new residents entering our care.

Key Responsibilities:

Develop and maintain strategic relationships with referral sources to increase facility occupancy. Coordinate the admissions process, including screening potential residents and reviewing clinical documentation. Conduct facility tours for prospective residents and their families, highlighting the quality of care at The Center at Arrowhead. Collaborate closely with the clinical and administrative teams to ensure all admissions meet the facility's capabilities and standards. Represent the facility at community events and healthcare networking functions. Track referral trends and provide regular census reports to management.

Qualifications:

Proven experience in healthcare sales, admissions, or community outreach (SNF experience preferred). Strong knowledge of Medicare, Medicaid, and private insurance reimbursement. Exceptional communication, negotiation, and interpersonal skills. Ability to work independently and manage a flexible schedule to meet the needs of referral partners.
