In this vital role, you'll be an important part of the patient care team, helping residents and families feel supported, informed, and cared for throughout their stay at Rio Vista Post Acute . Responsibilities Provide psychosocial support to residents and their families Assist residents with adjustment to their stay and change in health independence Participate in care planning and interdisciplinary team meetings Assist discharge planning and coordination of services Communicate with families, health care professionals, and community resources Advocate for residents and help address concerns or barriers to care Maintain accurate and timely documentation Help create a positive, supportive environment for residents and their families. Qualifications Bachelor's degree in Social Work or related fields Experience in a skilled nursing, post-acute, helalthcare, or similar settings preferred Strong communication and interpersonal skills Compassionate, organized and dependable Ability to work effectively as part of an interdisciplinary team Apply today and become part of the Rio Vista team.
