At UConn Health, this class is accountable for independently performing a full range of tasks in providing direct social work services to patients and families and includes both virtual and in person sessions. This position will work in the Adult Sickle Cell Unit on the campus of UConn Health in Farmington, CT.

THIS POSITION MAY BE FILLED AS SOCIAL WORKER I OR SOCIAL WORKER II. SUPERVISION RECEIVED

Works under the general supervision of an employee of higher grade.

SUPERVISION EXERCISED

May lead/supervise lower rank staff members and other staff as assigned.

EXAMPLES OF DUTIES

Assists with intake coordination. Performs psychosocial assessments on new and continued patients. Works independently and/or collaborates with staff in formulating and implementing treatment plans. Attends rounds, staff and team meetings and case conferences to provide social work consultation and/or referral for further intervention. Leads psychotherapy groups. Provides direct clinical social work with individual patients; conducts family meetings. Consults with other staff and community resources; engages patients and families in discharge planning. Records significant psychosocial data in patient charts. Prepares summaries and reports to other agencies. May provide court testimony as an expert witness. Conforms to institutional, department and professional standards. Maintains patient confidentiality. May participate in training residents and students. May supervise social work/public health students. Performs other duties as required.

MINIMUM QUALIFICATIONS REQUIRED KNOWLEDGE, SKILL, AND ABILITY

Considerable knowledge of principles, procedures, and techniques of medical and/or psychiatric social work services. Considerable knowledge of social, psychological, psychiatric, medical, economic, and legal factors influencing individual and group behavior. Considerable knowledge of group dynamics and case work methods. Considerable knowledge of community resources. Considerable interpersonal skills; oral and written communication skills Ability to plan and execute effective service programs meeting client needs. Ability to work autonomously to make decisions effecting patients and families. Ability to respond to crises under pressure and deadlines. Ability to work with chronically and terminally ill or physically impaired individuals and their families.

EXPERIENCE AND TRAINING.

General Experience:

SOCIAL WORKER I

A master's degree from an accredited school of social work

SPECIAL REQUIREMENTS SOCIAL WORKER I

Must possess and maintain licensure as a Master Level Social Worker (MSW) in the State of Connecticut. Must obtain the necessary clinical experience and supervised practice to be eligible to obtain licensure as a Licensed Clinical Social Worker (LCSW) in the State of Connecticut. May be required to travel. May be required to be bilingual.

SOCIAL WORKER II

A master's degree from an accredited school of social work and two (2) years of relevant experience in a psychiatric mental health setting working with psychiatric patients.

SPECIAL REQUIREMENTS SOCIAL WORKER II

Must possess and maintain licensure as a Licensed Clinical Social Worker (LCSW) in the State of Connecticut. May be required to travel. May be required to be bilingual.

PREFERRED QUALIFICATIONS

Experience working with a multidisciplinary team in a fast-paced environment. Sound understanding of complex disease and psychosocial environments with the ability to develop complex treatment plans that meets the psychosocial, spiritual, and mental health needs of the patient. Experience evaluating the need for transportation, STD, FMLA, or ADA and ensure patients have insurance, prescription assistance and need for housing with medical protections along with any other need that may arise.

SCHEDULE

Full time, 80 hours every two weeks, primarily 8:00 am to 4:30 pm with flexibility to meet patient and departmental needs, minor holidays as assigned, 30-minute unpaid meal break.
