Job Description

This is a perdiem social work role between three facilities: Mystic Healthcare in Mystic, Greentree Manor in Waterford, and Douglas Manor in Windham. We are a family-owned skilled nursing provider with 76 years of experience specializing in short-term rehabilitation and long-term care. Come join our dynamic team!
Hours:
Per-diem Social Worker Responsibilities Assist in developing and implementing policies and procedures for the identification of medically related social and emotional needs of the resident Participate in community planning related to the interests of the facility and the services and needs of the resident and family Participate in discharge planning, development and implementation of social care plans and resident assessments Interview resident/families to obtain social history Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social worker programs Maintain a quality working relationship with the medical profession and other health related facilities and organizations Coordinate social service activities with other departments as necessary Social Worker Requirements Bachelors Degree in Social Work or a bachelor's degree in a Human Services field, including but not limited to sociology, special education, rehabilitation counseling, and psychology Ability to make independent decisions when circumstances warrant such action Ability to relate information concerning a resident's condition We are an equal opportunity employer and value diversity.
