Social Worker Gemba Boise Boise, ID Job Details Part-time $34.26 - $37.65 an hour 22 hours ago Benefits Paid time off Employee assistance program Flexible schedule Qualifications Teamwork Bachelor's degree Psychology Human Services Social Work Full Job Description Gemba Boise is seeking a thoughtful, organized, relationship-driven Social Worker / Case Manager to join our team supporting neurodivergent young adults as they build more independent adult lives. Gemba is a coaching-based transitional living program serving young adults with autism, ADHD, anxiety, executive-function challenges, and related needs. Our approach is centered on helping young adults develop the capacity, skills, and ownership necessary to direct their own lives. This is not a traditional case-management role focused on doing things for clients. We are looking for someone who can coordinate complex systems while keeping the young adult at the center of the process, supporting increasing agency, self-advocacy, and independence over time. What You'll Do Serve as a primary point of contact for assigned Gemba residents Develop and maintain individualized plans based on each resident's goals, strengths, needs, and level of support Coordinate communication among residents, families, Gemba staff, therapists, psychiatrists, physicians, dietitians, and other outside providers Help residents navigate healthcare, community resources, appointments, education, employment, and other adult-life systems Participate in multidisciplinary team meetings and ensure important information is communicated across the team Track resident progress and identify areas where support may need to increase, decrease, or change Maintain timely, clear, professional documentation Support residents in developing self-advocacy, decision-making, organization, and follow-through Communicate with families while maintaining appropriate boundaries and protecting the young adult's growing autonomy Assist with transitions into, through, and out of Gemba Collaborate closely with coaches and program leadership to ensure services are coordinated and aligned Respond calmly and thoughtfully when residents experience periods of increased stress, dysregulation, or reduced capacity Who We're Looking For You may be a great fit if you: Have a bachelor's or master's degree in Social Work, Counseling, Psychology, Human Services, or a related field Have experience in case management, behavioral health, young-adult services, disability services, residential/transitional programs, or a related setting Understand autism, ADHD, executive functioning, and neurodiversity-affirming practice Are highly organized and able to keep track of multiple people, providers, appointments, and priorities Communicate exceptionally well with young adults, parents, professionals, and coworkers Can maintain warmth and empathy without over-functioning for clients Understand the difference between providing support and taking over Are comfortable with ambiguity and individualized approaches rather than relying on a one-size-fits-all program Value collaboration, accountability, curiosity, and respectful communication Experience working with neurodivergent young adults is strongly preferred. Pay:
