Social Services Assistant New Lebanon Rehabilitation and Healthcare Center At New Lebanon Rehabilitation and Healthcare Center , we are dedicated to providing compassionate, resident-centered care that enhances quality of life and promotes overall well-being. Position Overview The Social Services Assistant plays a key role in supporting the emotional, social, and psychological needs of our residents and their families. Working closely with the Director of Social Services and the interdisciplinary team, this individual helps residents adjust to their stay, supports discharge planning, and advocates for residents' rights and preferences. This position is ideal for someone who is compassionate, organized, and committed to making a meaningful difference in the lives of others. What You'll Do Assist the Director of Social Services in planning and implementing care plans that address residents' psychosocial needs Support residents and families with admissions, transfers, and discharge planning Help residents adjust to life in the facility through emotional support, counseling, and community resource referrals Maintain accurate documentation and records in accordance with state and federal regulations Collaborate with nursing, therapy, and administrative teams to promote a holistic approach to care Participate in care plan meetings and advocate for residents' preferences and well-being Provide support during challenging situations, including loss, grief, or adjustment difficulties Foster positive relationships with residents, families, and the broader care team What We're Looking For High school diploma or equivalent (required); degree or coursework in social work, psychology, or a related field (preferred) Experience in a healthcare or long-term care setting (preferred) Strong communication, empathy, and active listening skills Excellent organizational skills and attention to detail Ability to maintain confidentiality and professionalism at all times Compassionate attitude and commitment to improving residents' quality of life What We Offer Competitive pay and comprehensive benefits (medical, dental, vision, 401(k)) Generous paid time off and holiday pay Supportive leadership and a welcoming, team-oriented culture Opportunities for professional growth within the facility and organization The opportunity to make a daily difference in the lives of residents and families Ready to Join Us?
