ABOUT US
Our Minneapolis/St. Paul location serves clients and their families across the Twin Cities through multiple payer sources, including private pay, veterans programs, and county-funded care. We deliver personalized care that allows people to age in place with dignity.
WHAT YOU'LL DO
Build and maintain referral relationships with hospital discharge planners, case managers, and post-acute care coordinators across the Twin Cities metro Serve as the primary point of contact for hospital social work and discharge planning departments Identify and develop new referral sources with a focus on acute and post-acute care settings Close the feedback loop with referral partners on client outcomes — keeping those relationships strong and trust high Develop and execute a structured outreach cadence across target hospital systems in the metro
WHAT WE'RE LOOKING FOR
Direct experience working within hospital discharge planning, case management, or post-acute care coordination in the Twin Cities metro. This is a firm requirement Established professional relationships with hospital social workers, case managers, or discharge planning departments Working knowledge of private-pay home care and how it fits into the hospital-to-home transition Familiarity with Medicare/Medicaid and VA referral workflows is a strong plus Self-directed — comfortable owning a territory without daily oversight
COMPENSATION
Base Salary:
$65,000-80,000 depending on experience + Commission Commissions based on referrals Mileage reimbursement
