Position Overview We are seeking an experienced DD Provider Specialist Liaison to serve as our primary brand ambassador and intake liaison. In this hybrid role, you will bridge the gap between County Board SSAs, individuals, and families. While serving as our chief advocate for Ohio's Technology First initiative, you will also represent and market our comprehensive DODD waiver service lines—including Remote Support, Homemaker/Personal Care (HPC), Residential Respite, and Community Respite . You will represent our agency at County Board Provider Fairs, Family Expos, and Tech Demos , lead family intake meetings, conduct non-invasive home assessments, and maintain seamless referral relationships with County Board SSAs across Central Ohio. Key Responsibilities 1. Field Community Representation & Event Engagement (In-Person/travel)
County Board Fairs & Family Expos:
Set up and staff agency interactive booths at regional events (e.g., Franklin County DD Provider Fair at the Expo Center, Summit/Medina DD Tech Expos, local Family Connection Forums).
Live Hardware & Service Demonstrations:
Host hands-on demos showcasing assistive tech (fall radar, smart locks, medication carousels) while educating families on our HPC and Respite options .
Community Respite Networking:
Connect with local recreational centers, family support groups, and day programs to promote short-term respite care solutions for family relief. 2. Family & Client Intake (Multi-Service Conversions)
Empathetic Intake Conferences:
Conduct virtual and in-person discovery meetings with individuals, guardians, and team members to evaluate overall care needs.
Home Tech Assessments:
Perform environmental assessments to determine internet stability, cellular backup, and optimal sensor layouts for Remote Support. 30-
Day Trial Management:
Coordinate equipment delivery from our "Lending Library," review trial outcome data with families, and transition them into long-term waiver plans.
Service Line Cross-Selling:
Help families design blended care packages that pair Remote Support with targeted in-person HPC hours or short-term Respite care . 3.
SSA Outreach & System Navigation County Board SSA Engagement:
Deliver "Tech First & Respite Solutions" presentations during County Board SSA team meetings. Single
ISP & PAWS
Navigation:
Assist SSAs by providing compliant service documentation, safety protocols, and backup plans for Remote Support, HPC, and Respite services under
OAC 5123
rules.
Rapid Referral Triage:
Manage incoming intake traffic from our web portal and county inquiries, ensuring a 48-hour response turn-around.
Position Qualifications & Requirements Experience:
Minimum 2+ years of direct experience as an SSA, County Board Case Manager, Independent Support Broker, or Provider Liaison within the Ohio DD system.
