In this vital role, you will coordinate comprehensive patient care, facilitate discharge planning, and ensure seamless communication among multidisciplinary teams. Our clients are referred by the County and some are on a 5150 involuntary hold, some are conserved. We also manage the client court hearings for hold status. For this role, we are looking for only case managers with documented resume experience working in a locked psychiatric facility. Duties Coordinate inpatient and outpatient services, ensuring timely access to community resources and post-acute care options. Manage court hearings and appointments Collaborate with clinical teams to develop and implement treatment plans tailored to patients' mental health conditions and substance use disorders. Facilitate intake processes for new admissions, ensuring proper documentation and adherence to safety protocols within the locked psych unit. Lead discharge planning efforts by coordinating with families, community agencies, and healthcare providers to support ongoing recovery. Requirements Proven experience working within locked psychiatric units in a Psychiatric Health Facility (PHF), with a strong understanding of inpatient mental health protocols. Extensive knowledge of behavioral health practices, diagnostic evaluation techniques, and clinical case management principles. Ability to work collaboratively with multidisciplinary teams while managing multiple priorities efficiently in fast-paced settings.
