Position Summary The Medical Director is responsible for providing high-quality medical care to residents of Compass Park Skilled Nursing Facility (SNF) and Assisted Living (AL), while supporting clinical operations, regulatory compliance, interdisciplinary care coordination, and quality improvement initiatives. This role includes direct patient care, medical oversight of facility, serving as a resource to other providers within Meridian Medical Services, participation in facility meetings, administrative responsibilities, and shared on-call coverage with other providers. Essential Duties and Responsibilities Evaluate, diagnose, and treat residents within the Skilled Nursing Facility and Assisted Living settings. Conduct initial, routine, follow-up, and medically necessary visits in accordance with federal, state, and facility regulations. Develop, review, and update individualized care plans in collaboration with the interdisciplinary care team. Provide timely medical management of acute and chronic conditions. Ensure continuity of care through effective communication with nursing staff, specialists, hospitals, residents, and family members. Support smooth transitions of care during admissions, discharges, and readmissions. Medical Director Responsibilities Provide medical leadership and clinical guidance to facility staff, and other providers within Meridian Medical Services, regarding patient care and medical best practices. Participate in quality assurance, quality improvement, infection prevention, and performance improvement activities as requested by facility. Assist the facility in maintaining compliance with applicable CMS, state, and regulatory requirements. Review and provide recommendations regarding medical policies, procedures, and clinical protocols. Attend and actively participate in designated facility meetings, including but not limited to Quality Assurance and Performance Improvement (QAPI), care conferences, utilization review, infection control, and other medical staff meetings as requested. Attend clinical meetings as requested with Meridian Medical Services Leadership and Provider staff. Documentation and Regulatory Compliance Complete all required SNF documentation, orders, progress notes, certifications, recertifications, and other regulatory paperwork in accordance with CMS, state, facility, and payer guidelines. Ensure all resident encounters are documented accurately and completely. Complete medical charts, notes, and visit documentation on the same day services are rendered whenever possible and no later than required by applicable regulations and facility policy. Maintain compliance with billing and coding requirements and support accurate clinical documentation. Care Coordination Collaborate closely with the on-site nurse practitioner, physician assistant, or other advanced practice provider(s) to coordinate resident care. Provide clinical oversight, consultation, and support to the mid-level provider(s) practicing within the facility. Facilitate communication among nursing staff, rehabilitation services, specialists, pharmacies, and other healthcare partners. Ensure appropriate provider coverage and continuity of care during absences, vacations, or scheduling changes. Communicate scheduling changes promptly to facility leadership, clinical staff, and collaborating providers to minimize disruption in resident care. On-Call Coverage Participate in a shared call schedule with other designated providers. Provide after-hours, weekend, and holiday coverage as assigned through the provider call rotation. Respond to urgent clinical issues in a timely manner and provide appropriate medical guidance to facility staff. Coordinate resident care needs with covering providers to ensure seamless continuity of services. Professional Responsibilities Maintain all required licensure, certifications, DEA registration, and professional credentials. Maintain good standing with applicable hospitals, payers, and credentialing organizations as required. Foster professional relationships with residents, families, facility staff, and community healthcare partners. Adhere to all federal, state, facility, and practice policies regarding confidentiality, compliance, and professional conduct. Qualifications MD or DO degree from an accredited medical school. Current and unrestricted Indiana medical license. Board certification in Family Medicine, Internal Medicine, Geriatric Medicine, or related specialty. DEA registration and controlled substance prescribing privileges. Experience in skilled nursing, long-term care, geriatric medicine, post-acute care, or assisted living preferred. Strong leadership, communication, and interdisciplinary collaboration skills. Work Schedule Regular on-site clinical coverage at Compass Park Skilled Nursing Facility and Assisted Living in Franklin, Indiana. Schedule may vary based on resident census, facility needs, and coverage requirements. Average of 4-8 days per month. Attendance at required facility meetings and administrative functions. Participation in a rotating call schedule shared among credentialed providers.
