Position Overview The Lead Care Manager (LCM) plays a key role in the Enhanced Care Management (ECM) program by providing direct services, coordinating care, and supporting clients with complex physical, behavioral, and social needs. Under the direction of the ECM Supervisor and/or Clinician, the LCM works as part of an interdisciplinary Care Team to help clients access services, promote recovery and wellness, and navigate health and community systems. Responsibilities Conduct comprehensive assessments of client needs across physical health, mental health, substance use, oral health, trauma-informed care, housing, employment, and social supports. Develop and implement individualized Care Plans/Health Action Plans. Provide services in the setting that best meets the client's needs (home, community, office, or telehealth). Connect clients to community resources, social services, and support groups. With client permission, coordinate and advocate on their behalf with healthcare providers (e.g., primary care physicians). Utilize evidence-based practices including Motivational Interviewing, Trauma-Informed Care, and Harm Reduction techniques. Support Transitional Care Planning and collaborate with interdisciplinary team members. Conduct outreach and engagement efforts including calls, mailers, and field visits. Accompany clients to appointments when needed. Monitor client progress and update goals accordingly. Provide mental health promotion and arrange transportation as needed. Complete all required documentation within established timeframes. Participate in weekly team meetings, supervision, and required training (e.g., ACEs Certification). QualificationsMinimum Requirements 3+ years of relevant work experience. High school diploma or GED. Strong knowledge of local community resources; long-time community resident preferred. Ability to complete required training and ongoing education. Ability to work flexible hours, including occasional evenings/weekends. Preferred Qualifications Multilingual skills (preferred but not required). Prior experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role. Knowledge, Skills & Abilities Strong relationship-building and interpersonal skills. Ability to perform home visits and field outreach. Experience using EMR/EHR or care management software. Excellent organization and time-management skills. Ability to prioritize tasks and work independently or in a virtual team environment. Strong written and verbal communication skills. Sound judgment, active listening, and genuine compassion for others. Ability to maintain professional boundaries with clients and colleagues. Cultural competency and experience working with diverse populations. Comfortable with technology for documentation and communication.
