WHY AMIkids? At AMIkids Escambia Boys Base, we provide a safe, supportive, home-like environment for young men ages 10-18. Through positive relationships, individualized support, and therapeutic services, we help youth build the skills and confidence needed to successfully move forward. As a Transition Coordinator, you'll help youth prepare for their next chapter by coordinating educational, vocational, and reentry services. You'll work closely with youth, families, team members, and community partners to develop transition plans that support continued success after leaving the program. What You Will Be Doing Develop educational, vocational, and community opportunities for youth. Coordinate each youth's transition into and out of the program. Provide guidance on educational, career, and personal goals. Develop and maintain individualized Performance Contracts and transition plans. Coordinate assessments, orientations, guidance sessions, and transitional meetings. Connect youth and families with community resources and external partners. Conduct home visits when needed to support successful transitions. Maintain required youth documentation, reports, and daily logs. Serve as an advisor, mentor, and positive role model while ensuring youth safety and supervision. Transport youth and assist with additional program needs as required. Qualifications Bachelor's degree in Counseling, Social Work, Psychology, Special Education, Human Services, or a related field. Minimum one (1) year of related experience working with youth required; experience with at-risk youth preferred. Experience identifying and securing community resources preferred. Strong communication, organization, relationship-building, and case coordination skills. Ability to work effectively with youth, families, schools, employers, and community partners.
