This position supports management of PACE (Program-of-All-Inclusive-Care for the Elderly) participants including, quality, service, utilization and financial outcomes. Required qualifications:
Bachelor's Degree in Social Work, Business Administration, or a Health related field or equivalent education/experience Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 2 years of experience in operations, including staff supervision and budgeting; including at least one year experience working with the elderly.
Preferred qualifications:
Master's Degree in Social Work, Business Administration, or a Health related field or equivalent education/experience 1 year of experience in care management, population management and/or evidenced-based clinical care.
