Job Description
Key responsibilities include:
Coordinating and tracking behavioral health referrals Conducting outreach and follow-up with members and providers Maintaining accurate data entry and documentation in internal systems Preparing correspondence, reports, and meeting materials Supporting provider network coordination and departmental operations Delivering strong customer service and administrative support Qualifications Associate degree required; bachelor's degree preferred. Equivalent combinations of education and relevant experience may be considered. Minimum of two years of recent experience in behavioral health, healthcare coordination, or a related field. Demonstrated ability to exercise sound judgment, solve problems, and make decisions in a fast-paced environment. Strong verbal, written, and telephone communication skills. Experience supporting patient-centered or care coordination models is a plus.
