ASAP Position Summary West Hollywood Community Housing Corporation is an award winning developer of affordable housing the City of West Hollywood and the greater Los Angeles area. As part of the Resident Services Department, the Resident Service Coordinator (RSC) position will provide a wide array of wrap around services and programs to low-income seniors at our Movietown & Sierra Bonita apartments through- which enable these residents the capacity for residential stability, greater self-determination and independent living.
- The RSC works closely with the Property Management functions of the corporation to identify residents who need assistance; support resident retention; promotes educational and wellness activities for residents; fosters and builds community.
- Provides outreach
- prevention strategies and onsite programs to the resident community; works with the Resident Services department to develop the intervention strategies for "at risk" residents, including those facing barriers with hoarding; financial instability; isolation; or any combination of issues.
- Establish contact with existing residents and participate in conducting assessments and quarterly follow ups to ensure residents are accessing resources provided.
- Maintain a service provider referral network for senior residents of WHCHC's buildings
- establish effective relationships with agencies that provide psychological counseling, in-home support, health care services, case management, and financial support.
- Monitor the health and mental health status of residents and refer to mental health services or appropriate service providers. Enlist support of families when appropriate.
- Confer with on-site resident managers to review specific support needs of individual residents and general needs of the residential community.
- Assist new residents and property management staff with moving in and moving out of WHCHC buildings including a "Welcome Packet" for new residents.
- Work with WHCHC property management staff to mitigate lease enforcement issues or conditions that could lead to evictions.
- Organize and maintain resident files; maintain records for program compliance and reporting.
- Participate and attend community meetings and trainings pertinent to senior programs and development.
- Under supervision and working with the lead RSC, set monthly goals and targets, create timelines for achieving the targets and track progress towards achieving the outcomes. Regularly analyze progress and adjust program or goals as required.
- Implement and assist in the periodic evaluation of WHCHC's service coordination procedures and protocols.
- Other duties as assigned by Lead
RSC & DRS.
Knowledge, abilities, and skills- Knowledge of the aging process, elder services, disability services, legal liability issues relating to providing service coordination, substance abuse, and mental health issues among the elderly.
- Strong interpersonal and verbal communications skills.
- Ability to maintain effective working relationships with community social service and health services providers particularly those systems that serve the City of West Hollywood and the City and County of Los Angeles.
- Program assessment, planning and implementation of programs preferred.
- Excellent writing skills.
- Proficient skills and knowledge of databases (e.g., HMIS, ClientTrack) for documentation and reporting.
- Highly motivated and self-sufficient.
- Creative ideas for program development.
- Ability to handle a wide array of resident issues and provides conflict resolutions.
- Knowledge of working within the affordable housing world of supportive services.
- Ability to listen effectively.
- Team player.
- Sense of humor.
- Ability to lift up to10 lbs.
Minimum Qualifications:
- Qualified individuals should have a Bachelor of Social Work or degree in Gerontology (or a related field) required, Masters in Social Work preferred.
- Two years of experience providing case management, preferred.
- Valid California Driver's License.
- Russian speaker preferred.
- Proficiency in Microsoft software programs (Word/Excel/Outlook). Salary
- $60,000 + Health, Dental, Vision/403B/Vacation & Sick leave.
