HealthWorks Program Coordinator (RN or Licensed Social Worker) Macon County Health Department | Decatur, IL Help Improve the Health of Children in Foster Care Do you have a passion for improving the lives of children and families? The Macon County Health Department is seeking a compassionate and motivated HealthWorks Program Coordinator to oversee healthcare coordination for children in the legal care and custody of the Illinois Department of Children and Family Services (DCFS) across an eight-county region. This position plays a vital role in ensuring children receive timely medical care, connecting healthcare providers with caregivers, supporting case management staff, and strengthening the healthcare network that serves some of our most vulnerable youth. What You'll Do As the HealthWorks Program Coordinator, you will: Coordinate healthcare services for children in DCFS care throughout an eight-county service area. Build and maintain strong relationships with physicians, healthcare providers, community agencies, foster families, and DCFS staff. Conduct participant assessments, develop service plans, and coordinate referrals. Supervise and support HealthWorks case management staff. Ensure children receive required preventive healthcare and specialty medical services. Serve as the primary resource for healthcare providers and caregivers regarding the HealthWorks network. Provide education and training to healthcare providers, caregivers, community partners, and DCFS personnel. Complete program audits and maintain compliance with HealthWorks and State of Illinois requirements. Attend multidisciplinary meetings, trainings, and community collaborations. Promote quality healthcare, advocacy, and positive outcomes for children and families. Qualifications We're looking for someone who is passionate about serving children and collaborating across healthcare and human service systems. Required Qualifications Current Illinois Registered Nurse (RN) license or Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) in Illinois Strong communication, relationship-building, and organizational skills Experience collaborating with healthcare providers, community organizations, or child welfare agencies preferred Comfortable speaking to groups and providing training Strong computer and documentation skills Ability to work independently while managing multiple priorities Additional Requirements Valid Illinois Driver's License Reliable personal vehicle with current automobile insurance Willingness to travel throughout the eight-county service area: Clark Coles Cumberland Douglas Edgar Macon Moultrie Shelby Ability to work occasional evenings or weekends as program needs require Ability to lift and transport up to 30 pounds and travel in varying weather conditions Successful completion of a criminal background check, motor vehicle record check, and drug screening Why Join the Macon County Health Department?
