Job Description

GENERAL DESCRIPTION
The RESPITE Care Service Coordinator approves and coordinates services for eligible participants based on the availability of DHS Funding. RESPITE Care Service Coordinator will determine participant's satisfaction with services. The Respite Care Coordinator will aid individuals to obtain entitlement benefits, provide them with information about community resources, promote self-determination and facilitate community inclusion. The Respite Care Coordinator is directly responsible to the FSU Program Director.
RESPONSIBILITIES
Agency Responsibilities:
  • Attend all in-service meetings and other agency meetings as directed by the Program Director.
  • Be available to assist in general development of programs and services offered by the agency.
  • Comply with agency personnel policies.
  • Is directly responsible to the FSU Program Director.
  • Other duties as assigned.
Departmental Responsibilities for
Respite Care:
  • Responsibilities for overall operation of the RESPITE Care Services Program.
  • Take referrals for the RESPITE Care Program.
  • Meet with family to complete all forms needed for participation in the program.
  • Meet with families to review the Individual Care Plan and update paperwork.
  • Create and maintain RESPITE Care Service case files for recipients and providers.
Records are to be updated at least annually or as needed. Responsible for timely completion of all required paperwork and reports including but not limited to the following: Monthly Case Manager Report Monthly Mileage Report Chronological Documentation Complete monthly billing and submit to Program Director for approval. Complete Exit Summaries. Approve RESPITE hours monthly for families (work with the Program Director to Develop a monthly budget so that the annual RESPITE grant will last for a 12 month Period). Follow the RESPITE Care Policies and Procedures Guidelines. Ensure delivery of quality services. Follow FSU Policies and Procedures. Perform other related duties and assignments as required. Education (minimum requirements for education and experience) Bachelor's Degree or equivalent experience in social work, psychology, education, Nursing, rehabilitation counseling, or other or behavioral sciences, and/or vocation Rehabilitation services.
