In this role, you'll coordinate daily operations, assist with person-centered service plans, and ensure high-quality, compliant care. You'll collaborate with clinical teams, families, and community partners, track outcomes, and support continuous improvement. Responsibilities Coordinate day-to-day operations of community-based healthcare and social service programs Support person-centered service planning and ensure timely, accurate documentation Monitor program quality, compliance, and outcomes in line with Sevita and regulatory standards Collaborate with families, caregivers, community partners, and clinical teams to support participants Assist with scheduling, referrals, intakes, and resource linkage for individuals and families Collect and report program data; support audits, quality reviews, and continuous improvement efforts Provide outreach and education about Sevita's services within the community Contribute to a positive, inclusive culture that reflects Sevita's values of empathy and collaboration Required Skills Program coordination Case management documentation Knowledge of community health resources Regulatory and compliance knowledge (healthcare/social services) Data tracking and reporting Electronic health record (EHR) or client record systems Care plan development and monitoring Risk assessment and safety planning Microsoft Office (Word, Excel, Outlook) Stakeholder and partner relationship management
