We are seeking an organized and experienced Program Coordinator to oversee our service delivery, manage staff training, and ensure the highest quality of care for our clients. Job Overview We are seeking a DDA Program Coordinator is responsible for coordinating day-to-day program services for individuals receiving support through DDA programs. This position helps ensure that services are delivered according to individual plans, agency policies, DDA regulations, and quality standards. The Program Coordinator serves as a link between individuals, staff, families, providers, and management. Join us in creating positive change through innovative program development and community engagement! Responsibilities Coordinate daily operations of assigned DDA program services. Monitor implementation of each person's individualized service plan, goals, and support needs. Maintain regular communication with individuals, families, guardians, and outside providers. Ensure documentation is completed accurately and on time, including progress notes, incident reports, appointments, and program records. Assist with scheduling staff coverage and program activities. Monitor individuals' health, safety, and general well-being within the program. Support staff in carrying out services that promote independence, dignity, choice, and community inclusion. Assist with appointments, follow-up care, and coordination with medical and behavioral health providers. Track compliance with DDA requirements, licensing standards, and internal agency procedures. Report concerns, incidents, staffing issues, or service gaps to leadership promptly. Participate in person-centered planning meetings, team meetings, and trainings. Help with admissions, transitions, and discharges as needed. Conduct routine checks of files, records, and program operations for completeness and compliance Provide guidance and support to direct support staff as assigned. Experience Proven experience in volunteer management, supervising teams, or leading community programs. Strong background in public speaking, program development, project management, and social work. Demonstrated success in working with culturally diverse communities and underserved populations. Skills in marketing, social media management, data collection, and grassroots community organizing. Experience in addiction counseling or public health initiatives is a plus. Bilingual abilities are highly desirable to effectively serve diverse communities. Prior leadership roles involving team supervision, client communication, fundraising, or teaching are preferred. Strong organizational and communication skills. Ability to multitask and solve problems efficiently. Proficiency in tracking systems and documentation. A compassionate leader who is committed to quality care.
