This position requires a strong working knowledge of SD practices and COFR requirements, as well as responsibility for monitoring data, preparing reports, and ensuring adherence to applicable standards. The Coordinator works closely with the Clinical Service Director (CSD) to analyze data, adjust program components based on outcomes and fidelity measures, and support implementation of new service initiatives. The role requires professionalism, leadership, and a commitment to promoting independence, integration, and recovery. Individuals with lived experience with mental illness, intellectual/developmental disabilities, or substance use disorders are encouraged to apply. Essential Duties and Responsibilities General Program Responsibilities Prepare and maintain data reports using criteria established by the CSD. Ensure all required documentation is complete and available for auditors and regulatory reviews. Maintain compliance with agency policies and standards, including staff training and monitoring. Represent the agency on local committees as assigned. Establish program outcomes consistent with agency and contractual expectations; prepare monthly outcome reports. Organize and track information for audits and accreditation bodies such as CARF and MDHHS. Provide leadership within assigned program areas and cross-train staff as needed. Collaborate with the CSD to evaluate service needs and recommend future program changes. Self‑Determination (SD) Coordinator Responsibilities Serve as the agency's subject‑matter expert for all self-directed services. Provide education to consumers, families, and staff regarding SD benefits and responsibilities. Review monthly reports from Financial Management Services (FMS) to ensure medical necessity and appropriate service use. Train and monitor SD budgets, Plans of Service, and service authorizations. Act as liaison between consumers and FMS providers. Conduct quality and compliance reviews, including documentation, training, and service utilization monitoring. Clinical COFR Coordinator Responsibilities Serve as the agency's expert on COFR requirements and processes. Provide COFR education to consumers, families, and internal/external partners. Review documentation from partner CMHs to verify medical necessity and service use. Train and monitor service authorizations and Plans of Service for COFR cases. Serve as liaison between consumers and partner CMHs. Conduct compliance reviews related to documentation, training, and utilization. Education and Experience Bachelor's degree in a human services field approved by the State of Michigan for Care Manager roles. At least one year of experience working with individuals with mental health conditions. Knowledge, Skills, and Abilities Proficiency in Microsoft Office and standard office technology. Strong attention to detail, time management, and organizational skills. Effective written and verbal communication abilities, including accurate documentation. Strong critical thinking, analytical, and judgment skills. Ability to meet Medicaid supervisory eligibility requirements. Ability to support the implementation of new requirements from funding sources. Licenses and Certifications Certification as a QMHP, QIDP, CMHP, or appropriate Michigan licensure. Valid U.S. driver's license and Michigan-required vehicle insurance. Proof of U.S. citizenship or legal authorization to work in the United States. Maintain all required trainings and certifications.
