About Us Hope Against Trafficking is a nonprofit organization providing safe housing and restorative support for adult survivors of sex and labor trafficking in Southeast Michigan. The organization operates Magdalene Detroit, a 24-month, rent-free residential program designed to support survivors' physical, emotional, and economic transformation. Residents receive access to essential resources, trauma-informed services, life skills support, and a compassionate community focused on long-term healing, dignity, stability, and independence. Position Summary The Case Manager plays a vital role in supporting residents as they work toward healing and independence. Reporting to the Program Director, this position provides comprehensive, trauma-informed care coordination by managing medical, mental health, dental, vision, and prescription services; coordinating appointments; maintaining accurate documentation; developing individualized service plans; and advocating for residents with internal teams and community providers. This position operates within a Next Level Nonprofit framework , emphasizing ownership, measurable outcomes, proactive problem-solving, direct communication, and shared responsibility. Key Responsibilities 1. Medical & Clinical Coordination Coordinate all resident medical, mental health, dental, vision, and prescription services. Conduct initial health screenings and assessments at intake (within scope of role). Schedule, track, and coordinate all appointments; arrange transportation as needed. Follow up with providers regarding treatment plans, referrals, and continuity of care. Obtain and maintain updated medical records and documentation (within the past year when applicable). Collect discharge paperwork from all appointments and ensure timely upload to resident files. Monitor medications and maintain accurate medication logs in accordance with policy. Communicate urgent or significant medical and clinical concerns promptly to the Program Director. 2. Case Management & Resident Support Facilitate intake and orientation in collaboration with the Program Director. Develop, implement, and track Individualized Service Plans (ISPs) in collaboration with residents and leadership. Create and regularly update relapse prevention plans for each resident throughout all program phases. Conduct regular resident progress reviews (30/60/90 days) with the Program Director. Provide crisis intervention support to residents and program graduates as needed (within scope). Meet with residents individually as needed to support goal progression and stability. 3. Documentation & Compliance Maintain accurate, timely, and complete documentation, including: Case notes Incident reports Outcome tracking Medical and service records Ensure all documentation complies with organizational standards, ethical guidelines, and HIPAA requirements . Maintain organized and up-to-date resident case binders/files. 4. Advocacy & Collaboration Advocate for residents with external agencies, healthcare providers, and service organizations. Collaborate closely with the Program Director, Residential Manager, and Residential Coordinator to ensure integrated care. Coordinate with the Residential Manager for logistics and resident readiness while maintaining clear role boundaries. Communicate effectively across teams to ensure continuity and quality of care. 5. Crisis Response & On-Call Support Serve as primary on-call contact for clinical or medical emergencies on a rotating basis. Respond to crises in a trauma-informed, professional manner. Document and report all incidents promptly to the Program Director. 6. Program Participation & Operations Participate in staff meetings, trainings, workshops, retreats, and community events. Share responsibility for monitoring security systems (e.g., cameras) with program staff. 7. Spiritual Support (If Applicable to Program) Support voluntary spiritual activities, including prayer and Bible studies, in a manner that is respectful, trauma-informed, and aligned with resident choice and program guidelines. Qualifications & Requirements Bachelor's degree in Social Work ( BSW required ); MSW preferred . Minimum of 2 years of experience in trauma-informed care or related field. Knowledge of care coordination, crisis intervention, and working with survivors of trauma. Strong communication, organizational, and documentation skills. Understanding of HIPAA, confidentiality, and ethical practice standards . Valid driver's license, clean driving record, and reliable transportation. Ability to pass a background check and drug screening. Compensation Compensation is based on experience and qualifications.
