As the Regional Hub Program Coordinator, you'll serve as the operational lead for Region 6's participation in Missouri's Transformation of Rural Community Health (ToRCH) model. You'll help coordinate healthcare providers, community organizations, and social service partners to improve access to clinical, behavioral, and social care services throughout rural communities. Working closely with hospitals, Rural Health Clinics, behavioral health providers, EMS agencies, public health organizations, community-based partners, and regional leadership, you'll support seamless care coordination and strengthen the overall effectiveness of the Hub ecosystem. What You'll Do Serve as the primary spokesperson and operational lead for the Region 6 Hub. Convene Hub members and facilitate collaboration among healthcare providers, public health agencies, community organizations, and other regional stakeholders. Coordinate onboarding activities, participation expectations, and operational alignment across Hub partners. Support Leadership Board governance activities, including meeting coordination, agenda preparation, reporting, and follow-up activities. Manage the implementation of localized Hub programs, initiatives, and operational priorities. Monitor Hub performance, referral activity, program outcomes, and operational effectiveness. Oversee Region 6 Navigator utilization, workload distribution, and workflow performance. Support development, refinement, and implementation of referral pathways, care coordination processes, and operational workflows. Track, analyze, and report Hub operational, implementation, referral, utilization, and performance data using Community Information Exchange (CIE) platforms, dashboards, and other reporting systems. Identify operational barriers, workflow inefficiencies, service gaps, and coordination challenges while developing strategies for improvement. Facilitate collaboration among clinical, behavioral health, social service, and community partners to improve coordination across the care continuum. Coordinate communication among Hub partners, Regional Community Networks (RCNs), Regional Health Transformation Organizations (RHTOs), and other stakeholders. Escalate operational and coordination issues according to established Hub protocols and support resolution efforts. Promote continuous improvement and long-term sustainability of regional Hub operations. What You'll Bring Required Qualifications Strong project management and operational coordination skills. Exceptional communication, relationship-building, and facilitation abilities. Experience coordinating multiple stakeholders across organizations or systems. Strong organizational skills and attention to detail. Ability to analyze data, identify trends, and support performance improvement efforts. Ability to manage multiple priorities and deadlines in a dynamic environment. Commitment to community-centered, collaborative, and mission-driven work. Preferred Qualities Healthcare, public health, social services, or community development experience. Experience with care coordination, population health, community health initiatives, or value-based care programs. Knowledge of rural healthcare systems and community-based services. Experience working with healthcare coalitions, nonprofit organizations, public agencies, or cross-sector partnerships. Experience using dashboards, reporting systems, data analytics tools, or Community Information Exchange platforms. Leadership experience involving multi-agency collaboration and program implementation. What Success Looks Like Strengthen collaboration among Region 6 Hub partners and stakeholders. Regular meetings with community partners, healthcare organizations, and regional stakeholders. Occasional travel throughout the Region 6 service area. Public speaking, facilitation, and presentation responsibilities. Education, Certifications & Licensure Bachelor's degree in Healthcare Administration, Public Health, Business Administration, Social Work, Community Development, or a related field preferred. Experience in healthcare operations, public health, community engagement, care coordination, or program management preferred. Project management, public health, community health, or healthcare leadership certifications preferred. Valid driver's license and reliable transportation may be required for regional travel.
