Position Summary:
The Resource Specialist serves as a trusted guide and advocate for individuals with intellectual and developmental disabilities (IDD/DD) and their families. The Resource Specialist serves as a trusted guide and advocate for individuals with intellectual and developmental disabilities (IDD/DD), their families, and the broader network of professionals and community partners supporting them.
Supervisory Responsibilities:
None Essential Functions:
Non-Medicaid Caseload Management Maintain and manage a caseload of individuals with intellectual and developmental disabilities (IDD/DD) who are not eligible for Medicaid-funded case management services. Conduct needs assessments and develop individualized resource and support plans. Maintain contact with individuals and families to assess changing needs and provide ongoing guidance and support. Document services, referrals, contacts, and outcomes in accordance with agency policies and funding requirements. Resource Navigation & Information Provide individualized resource coordination, systems navigation, education, and advocacy for individuals and their support networks. Assist in identifying and accessing appropriate community, funded, or government resources. Educate families or individuals on available support and empower them to make informed decisions. Benefits & Systems Assistance Help individuals, families, and their support networks, understand and navigate complex systems including SSI/SSA, Medicaid, waivers, food assistance, and other public benefit programs. Assist families or individuals with applications, referrals, and accessing available supports. Guardianship Support Educate and support families through the guardianship process and available alternatives. Connect families with legal resources and community partners when appropriate. Education & School Collaboration Work collaboratively with families, schools, educators, and other support partners regarding IEPs, MPACT, Section 504 plans, transition planning, vocational rehab, and other educational supports. Serve as a liaison between individuals, families, educators, service providers, and other relevant stakeholders. Transition Planning Assist youth and families with transitions throughout the lifespan including: Early intervention School-age services Transition to adulthood Employment and post-secondary opportunities Independent living resources Community Connections Develop and maintain relationships with community agencies, healthcare providers, schools, employers, and other organizations. Promote awareness of available services and improve access to coordinated community support. Advocate for improved access to coordinated community support and services. Documentation & Reporting Maintain accurate documentation of services, referrals, contacts, assessments, and outcomes. Prepare reports and maintain confidential records in accordance with agency standards. Required Qualifications Education Bachelor's Degree Required Bachelor's degree in Human Services, Social Work, Psychology, Special Education, or a related field preferred Experience working with individuals with IDD/DD and their families preferred.
Preferred Knowledge in:
Missouri Division of Developmental Disabilities services Missouri Family Support Division-Medicaid, SSI/SSA Guardianship Missouri Vocational Rehabilitation DMH-funded programs Missouri Children's Division (when applicable) MPACT Missouri school special education law and IDEA Transition planning (ages 14-21) Person-Centered Thinking/Planning Required Skills & Abilities Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Strong organizational and documentation skills. Ability to maintain confidentiality. Ability to exercise sound judgment in emergency situations. Required Certifications & Licenses Valid driver's license with acceptable driving record Successfully complete: Agency Orientation Successfully complete and maintain all required agency training in accordance with agency policies, state regulations, and position-specific requirements. Physical Requirements Stand and walk for extended periods. Bend, stoop, kneel, squat, reach, push, and pull. Sit for extended periods while completing documentation or attending meetings. Operate agency vehicles. Work indoors and outdoors in varying weather conditions. Respond quickly during emergency situations. Must be free from the effects of mind-altering substances, including but not limited to alcohol, illegal drugs (including THC), or any medication or substance that impairs the safe performance of job duties. Working Conditions Residential homes, community settings, employment sites, agency facilities, and outdoor environments. Frequent local travel may be required. Evening, weekend, holiday, and flexible shifts may be required depending on program assignment.
