SOUTHERN MISSISSIPPI PLANNING AND DEVELOPMENT DISTRICT SENIOR SERVICES DIVISION JOB DESCRIPTION
Veteran Directed Care Program Coordinator Salary Range:
$50,000 CHARACTERISTICSOF THE JOB
Under the Supervision of the Senior Services Assistant Division Director, the VDC Program Coordinator is part of a team that supports the ongoing implementation and coordination of the VDC program.Activities will include:
assessing and orienting Participants and/or Authorized Representatives, educating and monitoring Participants' employees and providers, outreach and education, statistical information gathering and interpretation, and general oversight and quality assurances for third parties providing VDC case management services in areas outside of SMPDD's service territory, under contract withSMPDD. RESPONSIBILITIES
- Provides intake for the VDC program. This includes receiving successful screening results from the VAMC and initializing contact with the Veteran and/or authorized representative.
- Conducts screening, assessment and enrollment services for the VDC program.
- Works with Veteran to assure compliance to the Participant Spending Plan and budget.
- Conducts data collection to ensure quality assurance and program trends.
- Conducts annual re-assessments on all veterans. Provides annual quality assurance survey to be completed by Veteran or authorized representative.
- Conducts marketing and program promotion as requested by VAMC and other partner agencies.
- Assists in assessing client educational and orientation needs. Provides training, education, and outreach to clients.
- Refers veterans to community resources to assist with resolution of problems. Reviews and evaluates available resources and services from local agencies to provide social assistance for clients.
- Responsible for the oversight and quality assurance of the case management function performed by third parties under contract with SMPDD for areas outside of SMPDD's service territory.
- Oversee training of new VDC employees at SMPDD or third parties under contract with SMPDD to provide VDC case management services.
- Serve as the liaison between V AMC, AAA, and other partner agencies and SMPDD and other third party contractors.
- Participate in conferences, seminars, and meetings as needed to provide or receive information related to VDC, issues related to the Veteran population, and other professional development activities as approved by administration;
- Develops interagency and community communications and working relationships. Knowledgeable of community resources in all areas where services are provided. Performs public speaking as required;
- Any other duties as assigned by the Senior Services Assistant Director, Senior Services Division Director, and/or Executive Director.
