Education:
Bachelor's degree in a human services field (REQUIRED)Experience:

About A Caring Heart Case Management A Caring Heart Case Management provides compassionate, person-centered support to individuals with intellectual and developmental disabilities (IDD). Our services include community-based support, group home care, day programs, and respite services—each designed to promote independence, dignity, and inclusion. With a focus on advocacy, individualized care, and community engagement, we're proud to be a trusted partner in care. About this Position Each participant is served by a team of individuals comprised of habilitation technicians and a supervisor, with a clinical designation of Qualified Professional. We offer on-going training to ensure that our QPs are knowledgeable in the most current therapeutic approaches and resources for the habilitation of individuals with intellectual/developmental disabilities. As a result, the individuals and families we support receive the services that best meet their needs. Our QPs assist the family with finding the best support staff for their loved one. The family and the individual served choose the staff person(s) to provide direct support services to them. QPs stay closely involved to ensure quality services are being provided. Our QPs supervise all services provided to the consumer through monthly observations at home, work, and/or in the community. Our QPs assist with developing the Individual Support Plan for accessing and using resources that can meet identified needs based on the individual's and family's strengths. Our QPs develop short range goals that the individual/family wants to work on in order to increase the individual's independence. Our QPs work with the care coordinator and other team professionals to ensure family needs are being met. Please describe your experience in a supervisory role, particularly as it relates to overseeing Direct Support Professionals (DSPs). Include details about your responsibilities, the number of staff you supervised, and how you supported their performance and development. Are you currently credentialed as a Qualified Intellectual and Developmental Disabilities Professional (QIDDP or QP) under North Carolina state regulations? If so, please include details such as your qualifying degree, relevant experience, and the date you were credentialed. This position may require physical tasks such as lifting, pushing, or pulling. Are you able to safely lift and carry up to 25 pounds on a regular basis and up to 100 pounds occasionally, without assistance? Please describe the populations you have supported in your previous roles as a Qualified Professional (QP) or Direct Support Professional (DSP). Include any experience with specific age groups, diagnoses, or support needs. Are you comfortable assisting individuals with Activities of Daily Living (ADLs) after receiving training? These tasks may include but are not limited to: Personal Care, Mobility Support, Meal Preparation & Feeding, Toileting & Continence Care, and/or Household Tasks.
