Paid sick/vacation time, holidays, 403(b), health, dental About Us The Brain Injury Association of New Hampshire is seeking a compassionate, organized, and motivated individual to join our dynamic team of professionals dedicated to supporting individuals living with brain injury, along with their families and the providers who serve them. Position Overview This role provides direct and indirect support services, resource coordination, outreach, and education to individuals and families impacted by brain injury. The position is fully in‑office at our Concord location and reports to the Director of Programs and Services . Key Responsibilities Direct Services Provide support to assigned individuals and families living with brain injury. Meet with individuals/families to develop person‑centered plans of care. Gather, review, and organize records and relevant information. Identify concerns, service needs, and requested supports. Conduct ongoing assessments and update planning documents. Provide information, education, and resources to individuals, families, and professionals. Serve as liaison to BIANH support group facilitators; track data and maintain scheduling. Assist with Brain & Spinal Cord Injury Advisory Council (BSCIAC) activities, including public hearings and surveys. Prepare and distribute informational materials to survivors and families. Maintain resource packets, library materials, and literature. Enter resource information into the office database; file and photocopy materials. Provide education to schools and community groups on brain injury prevention. Update and maintain the Resource Library. Conduct family outreach calls. Provide back‑up coverage for incoming phone calls. Support Case Managers with resource identification. Provide materials and resources to Support Group Facilitators as requested. Indirect Services Consult with agencies and providers on brain injury issues to promote equitable access and treatment. Deliver training and outreach to community agencies. Maintain confidentiality and adhere to ethical standards of practice. Qualifications Bachelor's degree in Human Services, Counseling, Social Work, Psychology, Sociology, or related field. Understanding of disability issues. Strong written and verbal communication skills. Proficiency with Microsoft Office; experience with electronic health records preferred. Ability to work independently and manage multiple tasks. Valid driver's license, reliable transportation, and adequate vehicle insurance. Must pass background checks, including criminal, DCYF, and BAAS state registry checks. Compensation Starting at $21/hour. Higher compensation may be offered based on relevant experience and qualifications.
