Job Description
Overview We are seeking a dynamic, compassionate, and dedicated Program Coordinator to oversee and support Community-Based and Day Habilitation Services for individuals with developmental disabilities. This position offers an opportunity to make a meaningful impact by coordinating person-centered services, supporting individual goals, promoting independence, and creating opportunities for meaningful community participation, social engagement, and skill development. The ideal candidate will have strong leadership and organizational skills, experience working with individuals with developmental disabilities, and a genuine passion for supporting people to live fulfilling and inclusive lives. The Program Coordinator will work closely with individuals, families, direct support professionals, and other service providers to ensure quality services, effective communication, and compliance with applicable requirements. We are looking for someone who is compassionate, dependable, organized, and committed to person-centered care , with the ability to build positive relationships and create an environment where individuals can learn, grow, participate in their communities, and achieve their personal goals. Duties; The Program Coordinator will be responsible for the overall coordination, supervision, and quality monitoring of Day Habilitation, Community-Based Supports (CBS), and Individual Supports (IS). The position will ensure that services are delivered in accordance with individuals' ISPs, approved authorizations, NJ DDD requirements, and agency policies. The Program Coordinator will work closely with individuals, families/guardians, Support Coordinators, DSPs, and other members of the individual's support team to promote quality outcomes, compliance, and continuity of services
