Job Description
The Support Coordinators will engage with individuals and their families through the New Jersey DDD Supports Program. They are responsible for developing plans that ensure the needs, choices, and interests of the individual are addressed and that their life opportunities are expanded. Support Coordinators assist with the choice of services and supports, coordinate their provision, and monitor their delivery. The Support Coordinator will promote individuality and choice. This position will be performed consistently with the values, principles, and philosophy of the Division of the Development Disabilities and all applicable policies and procedures. The Support Coordinator will have a strong foundation of experience in providing support coordination or services to people with cognitive disabilities, and a strong belief that people with disabilities can achieve outcomes, have value and contribute to their community.
