Youth Care Management Program Coordinator 3.1 3.1 out of 5 stars 55 Horizon Drive, Huntington, NY 11743 $70,000 a year WellLife Network 120 reviews $70,000 a year The Youth Care Management (YCM) Program Coordinator works as part of an administrative team to oversee the operations of the Youth Care Management Program. The YCM Program Coordinator is responsible for establishing and implementing agency and Health Home policies and procedures, maintaining operations and clinical functions of the program. The YCM Program Coordinator directly supervises the Youth Care Management team and reports to the YCM Program Director. Responsibilities include but are not limited to: Demonstrates knowledge of the regulations of the Health Home Serving Children Program/ System. Demonstrate knowledge and proficiency with Health Home Serving Children systems such as the Health Commerce System, MAPP, UAS NY, Foothold, and CANS NY. Maintain collaboration with supportive services such as CFTSS, HCBS, CSPOA, etc. Demonstrates knowledge and understanding of the policy and procedures of the lead Health Home. Oversees the recruitment and hiring process of staff. Provide ongoing supervision of staff. Development, implementation, and maintenance of specific program policies and procedures that will ensure efficacy, efficiency, and quality of services. Responsible for ensuring compliance with all DOH, Medicaid, and Health Home regulations. Responsible for revenue monitoring, creative problem solving, projections/ forecasting based on current processes, align processes to ensure resolution of issues and report to leadership. Provide ongoing administrative and/or clinical supervision of the care of the clients. Demonstrate skills related to marketing. Report to the Coram location as needed and requested for program meetings and events. Attend/facilitate meetings and trainings as requested. All duties as assigned.
