This is a full-time, in-office position within an established and thriving organization with significant growth potential.
POSITION SUMMARY
The NHTD Coordinator is vital role dedicated to overseeing and managing all aspects of participant enrollment, service initiation, compliance monitoring, and ongoing program coordination related to the Nursing Home Transition and Diversion Waiver Program.
EDUCATION AND EXPERIENCE
Prior NHTD coordination experience is mandatory Thorough knowledge of NHTD rules, regulations, and waiver requirements New York State Department of Health guidance is also required
QUALIFICATIONS
Strong understanding of NHTD compliance requirements Experience working with Service Coordinators, RRDCs, and community providers Strong organizational and communication skills Bilingual English and Spanish preferred but not mandatory
DUTIES AND RESPONSIBILITIES
Coordinate participant intake and enrollment Ensure regulatory compliance Monitor authorizations and service implementation Communicate with stakeholders Maintain accurate records Support audits and quality assurance Resolve service issues Assist with departmental development and growth
AVAILABILITY REQUIREMENTS
Monday -
Friday:
9am - 5pm
•Must be available after normal business hours as well to address urgent participant needs, service disruptions, and program-related emergencies.
