Plan and execute rural health initiatives aligned with grant objectives (e.g., Rural Health Network Development program) Coordinate workshops, trainings, and outreach activities focused on health education and awareness Ensure all program activities align with state and federal guidelines Community Engagement & Outreach Build and maintain partnerships with healthcare providers, schools, community organizations, and local agencies Conduct outreach to underserved populations to increase access to services Represent the organization at community meetings, events, and coalitions Health Education & Awareness Organize and facilitate workshops on topics such as: Preventive care Chronic disease management Mental health awareness Access to healthcare resources Develop culturally appropriate educational materials Data Collection & Reporting Track program metrics, participation, and outcomes Maintain accurate records for reporting requirements Assist in preparing reports for funders and state agencies Ensure compliance with all grant deliverables and deadlines Collaboration & Network Development Strengthen partnerships across the rural health network Support coordination between healthcare systems and community-based organizations Identify gaps in services and recommend solutions
