Our mission is to love, care for, and accept young people by guiding their development through the love of God, academics, life skills, and health/exercise.
Role:
The KIND Care Program Coordinator is responsible for partnering with CareSource to provide access to healthcare and health-related services to the KIND kids and families living in East Dayton. KIND will act as an extension of the CareSource team to create connection between East Dayton families and CareSource. KIND will also be responsible for coordinating with families, scheduling appointments, and transportation. In addition, KIND will report KIND Care Program progress and outcomes to CareSource. KIND's ultimate desired outcome is to create opportunities for our KIND kids to improve their health resulting in improved school attendance while developing the necessary skills to increase their likelihood of success as an adult. This position reports to the KIND's Executive Director (ED). Hours Part-time (15-20 hours per week)
Responsibilities Program Operations:
  • Work with the ED to maintain efficient operations and relevant procedures to ensure KIND Care Program is effective and aligned with the mission.
  • Work with KIND staff to establish a Healthcare "Track" in the KIND curriculum.
  • CareSource Relationship ◦ Function as the primary interface with CareSource. ◦ Foster and maintain an active relationship with the CareSource support team and social worker. ◦ Work with the CareSource support team and social worker to gain a thorough understanding of the CareSource programs available to the KIND families.
  • Engage Families Monthly ◦
Family Healthcare Packages:
Procure, track, and manage healthcare supplies used to create and distribute monthly family healthcare packages. ◦
Family Check-in:
Create a standard process to collect family information that will help KIND continually improve the program.
  • Plan Quarterly KIND Care Events that will educate and encourage families to participate in the KIND Care program.
  • Coordinate annual dental and vision check-ups for KIND Kids and their families.
  • Coordinate medical check-ups for KIND Kids and their families.
  • Manage program budget within allocated funding and report to ED and KIND Board.
  • Develop and report program measures and outcomes against goals on a monthly, quarterly, and annual basis to KIND and CareSource.
  • Support KIND Development Committee in renewing grant annually.
Measures & Goals:
2024 will be a baseline year to improve upon in future years. Goals will be finalized with the KIND Care Coordinator and KIND Board and report accordingly to CareSource. Desired Outcomes 1. Improved health of KIND Kids and their families. 2. Reduced chronic absenteeism from school. 3. Reduced disciplinary actions at school. 4. Increased use of CareSource services. 5. Discover relationships between healthcare measures/outcomes and school performance, social impact, self-esteem, etc.
Desired Skills/Experience:
  • Demonstrated experience working in a small non-profit organization.
  • Proven leader, mentor, and self-starter.
  • Experience managing projects & superior organizational skills.
  • Understands the need for attention-to-detail.
  • Solid written, oral and presentation skills.
  • Practical experience using office automation tools; preferably Microsoft Office and Google Suite.
  • Willingness to learn how to use non-profit fundraising software.
  • Humble enough to dig-in with the team to get the job done.
  • Experience working with underserved populations and youth.
Pay:
$25,000.00 - $30,000.00 per year
