Job Description
Broadfield Care Center, a 48 bed intermediate care facility, located in Madison, OH is looking for a dedicated Residential Coordinator to join our team! The position is 2nd shift (2p-10p) The Residential Coordinator is a pivotal position for the home. This individual holds primary responsibility for areas of program, home environment and management for the individuals. In these areas, the Residential Coordinator should be involved in all assessments, planning and implementation of services and supports. Finally, s/he should assist in direct training, follow up and coaching with service staff, keeping the QMRP informed at all times. The person in this position has the responsibility to further coordinate a variety of services including but not limited to: monitoring of individuals' personal money, recreation, health services, family involvement, education/vocational programs and all other supports/services necessary to ensure a good quality of life. The Residential Coordinator is responsible to assist the QMRP in maintaining consistency in the provision of supports and services to the individuals, and in communicating with staff and team members.
