(HRC) is dedicated to providing high-quality services and support for individuals with intellectual disabilities. Our Home-Based Waiver (HBW) program helps individuals live independently while receiving the necessary assistance to thrive. Job Summary As a Home-Based Services Program Specialist , you will play a vital role in supporting individuals and families within the HBW program. This position involves developing, coordinating, assessing, and implementing Individual Support Plans (ISPs), ensuring quality services, and overseeing HBW employees. You will work directly with individuals to enhance their independence while collaborating with families and service providers. Key Responsibilities Provide case management support for individuals and families in the HBW program. Develop, implement, and monitor Individual Support Plans (ISPs) to meet program requirements. Conduct scheduled home visits and document case notes. Coordinate services with other providers and community resources. Review and approve daily service notes and case documentation. Assist with recruiting, hiring, training, and managing service providers. Participate in ISP team meetings and help develop annual program goals. Assist families in accessing additional resources, such as assistive technology or home modifications. Ensure compliance with all agency policies, safety measures, and state regulations.

Qualifications Education & Experience:

Master's degree in a related field + 1 year of experience working with individuals with intellectual disabilities OR Bachelor's degree in a related field + 2 years of experience Associates degree in a related field with + 3 years of experience

Additional Requirements:

✔ Must be 21 years or older ✔ Valid PA Driver's License with an acceptable driving record ✔ Ability to pass required background checks (PA Criminal, Child Abuse, Federal) ✔ Successful completion of CPR, First Aid, and Crisis Prevention Intervention (CPI) training within required timeframes ✔ Ability to complete ACRE Online Training within 180 days of hire Why Join Us?
