Responsibilities:

Provide direct patient care to residents on the evening shift, including administering medications, wound care, and other nursing interventions Develop and implement individualized care plans in collaboration with the interdisciplinary team Monitor and report resident conditions, progress, and concerns to the nursing staff and other healthcare professionals Collaborate with other healthcare professionals to ensure continuity of care and effective communication Participate in quality improvement initiatives to enhance the overall quality of care and services Maintain accurate and thorough records of patient care and experiences About the

Requirements:
