Duration:
Longterm Contract Shift hours: 8am - 4pm with 1/2 hour unpaid lunch MAT Program Specialist - Job Overview The MAT (Medication-Assisted Treatment) Program Specialist is responsible for providing case management, counseling, care coordination, and reentry support services to individuals involved in the correctional system. This role focuses on supporting individuals with substance use disorders by coordinating treatment plans, facilitating community referrals, and ensuring continuity of care during reentry. Key Responsibilities Coordinate with SCI and BCC MAT teams regarding reentrants requiring detoxification and determine eligibility for MAT services. Assist with community MAT referrals, including scheduling treatment and medication appointments. Ensure assigned individuals receive appropriate services, benefits, and support according to treatment plans. Track and maintain records for individuals released on MAT programs using spreadsheets and databases. Attend weekly Vivitrol conference calls, treatment meetings, and required trainings. Document all case activity and client interactions in systems such as Captor and Sapphire. Conduct assessments for reentrants with positive drug screens and refer individuals to appropriate treatment services. Complete GPRA documentation requirements (initial, 6-month, and discharge assessments). Meet with reentrants at correctional facilities, parole offices, treatment centers, and community settings as needed. Communicate treatment planning updates with center staff, SCI staff, parole officers, and treatment providers. Provide social work, counseling, and case management services to improve psychosocial functioning and successful community reintegration. Participate in multidisciplinary treatment team meetings and discuss psychosocial assessments and family history. Ensure compliance with DOC policies, MAT treatment frameworks, HIPAA, and confidentiality regulations. Qualifications Bachelor's degree in Social Work, Human Services, or a related field required. Experience in substance use treatment, behavioral health, case management, or correctional settings preferred. Proficiency in Microsoft Excel and case documentation systems. Strong written and verbal communication skills. Ability to build effective working relationships with clients, staff, and community partners. Required Knowledge & Skills Knowledge of social work principles, counseling techniques, and case management practices. Understanding of substance use disorders, behavioral health, and rehabilitation resources. Familiarity with HIPAA regulations and confidentiality standards.
