MAT Program Specialist Advanced Level Quality Patient Centered Services LLC (QPCS) is seeking a knowledgeable and compassionate MAT Program Specialist to support recovery and reentry services at SCI Fayette. This position coordinates medication-assisted treatment services, connects individuals with community resources, and provides counseling and case-management support to promote continuity of care and successful community reintegration.

Advanced Level Opportunity The successful MAT Program Specialist can perform assigned responsibilities independently and serve as a knowledgeable resource when challenging questions arise regarding medication-assisted treatment, substance-use recovery, counseling, and case management. Key Responsibilities Assess individuals' psychosocial strengths, treatment needs, eligibility for services, and barriers to recovery. Coordinate MAT referrals and schedule community appointments for medication, counseling, and treatment. Provide social work, counseling, and case-management services consistent with individualized treatment plans. Meet with individuals following positive drug screens and make appropriate treatment referrals. Communicate treatment plans and service needs to SCI staff, parole personnel, community providers, and other authorized team members. Document all contacts in required systems and maintain accurate program spreadsheets and databases. Complete required GPRA assessments, including initial, six-month, and discharge evaluations. Participate in multidisciplinary treatment meetings, conference calls, training, and other required program activities. Follow all DOC MAT and alcohol-and-other-drug treatment policies, procedures, and confidentiality requirements. Minimum Qualifications Bachelor's degree in Social Work or a related human-services field. Knowledge of professional social-work, casework, counseling, and behavioral principles. Understanding of substance-use disorders, medication-assisted treatment, behavioral health conditions, and community resources. Knowledge of counseling techniques and interventions used to improve psychosocial functioning. Understanding of HIPAA, state confidentiality requirements, and applicable DOC policies. Proficiency with Microsoft Excel and electronic case-management or documentation systems. Strong written and verbal communication skills. Ability to establish and maintain effective professional relationships. Ability to work professionally in a secure correctional environment.
