The Heart Failure Program Coordinator :

Coordinates and facilitates all components of the heart failure program. Directs the program's Quality Improvement components required to maintain accreditation standards and is responsible to maintain effective lines of communication between appropriate accreditation agencies. Ensures that the programming is patient-centered, effective, and efficient. Efforts are directed at optimizing the care received by the patient. Ensures integration of other disease programming as appropriate to address chronic comorbid conditions. Promotes collaborative care practices and participates in establishing partnerships with the physicians and the entities of the Reading Health System to provide identified related services.

Work Schedule:
