Accident Insurance, Critical Illness Insurance Position Overview:
Geriatric Support Services Coordinator (GSSC) participates in the Senior Care Options (SCO) primary care team to develop, authorize, coordinate, and monitor consumer-centered community-based services to an assigned number of United Healthcare SCO members in collaboration with the United Healthcare SCO Nurse Case Managers and under the supervision of the SCO Program Manager/Supervisor.
Position Responsibilities:
The Geriatric Support Services process begins with a comprehensive needs assessment, completed as part of an interdisciplinary team. Requires a thorough knowledge of available community services, an understanding of both formal and informal support systems, and an ability to interface and collaborate with those systems to ensure the elder's needs are met. The position entails extensive coordination, follow-up, and feedback to clients and services providers, as well as recordkeeping incompliance with regulations. This position shall not provide GSSC services for any other SCO contractor.
Qualifications:
Bachelor's Degree with two (2) years of elder case management, at least one (1) year must be in a setting that provides health care services to people over sixty-five (65) years of age required Excellent computer skills with proficiency in Microsoft Word, Excel, and PowerPoint as well as managing social media platforms. Exceptional communication skills including writing, public speaking and active listening Customer service, communication and recordkeeping experience required Ability to manage multiple concurrent tasks and projects with strict attention to detail Solid organizational, time management and project management skills with the ability to meet deadlines required Strong collaborative skills that allow for fostering and promoting business relationships and partnerships within the community Comfortable working with people from diverse backgrounds Hours Full Time, 32 hours per week Pay Range $22.50
