Chosen Home Care Services LLC is seeking a qualified CANS Assessor to support our OhioRISE program on an as-needed/contract basis. This is a great opportunity for an experienced professional looking for flexible, supplemental work in the Columbus and Springfield, Ohio, areas. Responsibilities Complete CANS assessments for referred youth Interview youth, parents, and caregivers Identify strengths, needs, risks, and support needs Complete documentation accurately and on time Enter required information into the appropriate CANS system Communicate professionally with families, care coordinators, and agency staff Maintain HIPAA compliance and confidentiality Qualifications Current Ohio Children's Initiative CANS certification Knowledge of the OhioRISE program Experience working with children, adolescents, and families Strong communication and documentation skills Dependable and able to work independently Previous CANS assessment experience preferred
