RESPONSIBILITIES

JOB SUMMARY
Coordinates and tracks program activities for the Remote Patient Monitoring (RPM) Program, including monitoring deliverables, maintaining program documentation, compiling program data and reports, and supporting the Operations Manager in the administration and continuous improvement of culturally grounded outpatient services for patients with chronic conditions. Reviews payer requirements and recommends changes to operational workflows, and escalation pathways. Coordinates, develops, writes, manages timelines and submits grant proposals. Develops departmental workflows and staff training for efficiency and compliance. Represents the program within The Queens Health Systems and in the community.

N/A III. TYPICAL WORKING CONDITIONS
Not substantially subjected to adverse environmental conditions. Work involves extensive off-site activities, including home and community-based visits with patients. Must have reliable transportation in order to make visits to sites where patients reside. May be exposed to negative responses from patient or family.
IV. MINIMUM QUALIFICATIONS
A.
EDUCATION
Bachelor's degree required, preferably in business or a health related field; or four (4) years health care experience may be substituted for the educational requirement.
B. CERTIFICATION AND LICENSURE
Valid U.S. driver's license required in accordance with Hawaii state law rules and regulations. Driver's abstract or evidence of good driving record. Current BLS certification.
C. EXPERIENCE
In addition to the educational requirement, two (2) years of relevant healthcare/social services experience; or one (1) year of healthcare/social services experience. Previous experience working with individuals with behavioral health and/or substance use issues preferred. Prior experience working with Native Hawaiian and other underserved populations preferred.
