Job Responsibilities Program Compliance & System Coordination:
Partner with DHHS Division, Beacon House, and Coordinated Entry staff to ensure 100% program compliance across Supportive Housing, UHIMS, DSPD, and related initiatives.
UHIMS & Coordinated Entry Management:
Oversee Utah Homeless Information Management System (UHIMS) data collection, entry, and reporting accuracy while coordinating client referrals and housing placements through the local Coordinated Entry system.
DSPD Client Lifecycle Management:
Coordinate DSPD housing and support services from initial intake, eligibility determination, and waitlist tracking through active program enrollment, ongoing service delivery, and disenrollment.
Operations & Accreditation Oversight:
Ensure all DSPD and Supportive Housing tasks, Case Management, and Peer Support services strictly adhere to daily, weekly, monthly, quarterly, and annual CARF standards, DHHS regulations, and Beacon House policies.
Staff Scheduling & Service Logistics:
Collect and document availability and schedules for Housing Case Managers; coordinate client transportation, and Residential Support Services in alignment with individual levels of care.
KPI Tracking & Billing Alignment:
Monitor scheduling adherence and key performance indicators (KPIs) for the Housing Coordinator and House Managers, delivering regular performance statistics directly to the Billing department.
Crisis Response & On-Call Coverage:
Manage team coverage and directly respond to crisis situations or staffing gaps when scheduled housing personnel are unavailable.
Direct Client Care & Advocacy:
Support individuals with daily living needs alongside Treatment and Residential Teams, advocate for client goals, and assist them in navigating healthcare and social service systems within certified Case Management and Peer Support scopes.
Documentation & EMR Maintenance:
Maintain precise records by logging every client interaction in the EMR within 24 hours, consistently meeting all assigned documentation KPIs.
Essential Skills & Qualifications Program & System Expertise:
Proficiency in housing data management systems (specifically
UHIMS/HMIS
) and a working knowledge of Utah Division of Services for People with Disabilities (DSPD) intake, waitlist, and funding processes.
Compliance & Accreditation Auditing:
Thorough understanding of CARF standards and state DHHS regulations to ensure 100% compliance across all program documentation and service delivery.
Crisis Prevention & Intervention:
Strong de-escalation capabilities and decisive problem-solving skills to manage unexpected behavioral or operational crises safely and effectively.
Team Leadership & Staff Supervision:
Natural ability to guide, mentor, and motivate House Managers and Housing Coordinators, fostering accountability while driving high performance across team KPIs.
Basic Project Management:
Proven capacity to organize workflows, manage timelines, align cross-functional tasks (such as billing and compliance audits), and coordinate multi-faceted program initiatives.
Resource Logistics & Scheduling:
Excellent organizational skills to oversee house manager schedules, coordinate client transportation, and balance level-of-care requirements with billable services.
