Summary/Objective:

Care Eleven Health is seeking an experienced Rural Health Transformation Program Manager to lead the planning and implementation of complex government funded healthcare initiatives. This role is ideal for a hands-on program leader with demonstrated experience in government grant administration, formal project management, cross functional coordination, budget oversight, risk management, and executive reporting. The successful candidate will bring the structure, judgment, and accountability needed to translate award commitments into executable plans and measurable outcomes. This position is anticipated to support one or more Rural Health Transformation initiatives funded through a state administered federal program. Care Eleven Health has applied for potential funding through the Louisiana Department of Health Rural Health Transformation Program, which is supported by the Centers for Medicare & Medicaid Services. Depending on the final approved award scope, this position may oversee one initiative or coordinate multiple related workstreams. The position is grant funded and subject to final award approval and continued funding. Upon final award approval, the Rural Health Transformation Program Manager will lead the operational planning and implementation of Care Eleven Health's Rural Health Transformation initiative or initiatives. The Program Manager will establish implementation plans, budgets, schedules, reporting controls, risk management processes, and accountability structures while coordinating contractors, vendors, internal leaders, and external partners. This is a hands-on leadership role without direct supervisory responsibility for Care Eleven Health employees. The position requires the ability to independently manage complex government funded work and keep leadership informed of progress, spending, outcomes, risks, and required actions.

Essential Functions & Responsibilities:

Translate award agreements, scopes of work, budgets, and milestones into integrated implementation plans with accountable owners, deadlines, dependencies, and measurable deliverables. Establish and maintain program management controls, including work plans, integrated schedules, critical path dependencies, responsibility matrices, decision logs, risk and issue logs, change control processes, milestone tracking, and executive status reporting. Lead cross functional work involving contractors, vendors, internal functional leaders, and external partners by establishing clear deliverables, deadlines, performance expectations, and accountability processes. Coordinate applicable program workstreams, including school onboarding, partner participation, staffing, vendor support, data readiness, and other implementation activities included in the awarded scope. Coordinate contractor and vendor onboarding, scopes of work, deliverables, timelines, performance expectations, documentation, and issue resolution in partnership with organizational leadership and applicable functional owners. Partner with finance and leadership to administer and monitor government award budgets, spending, staffing assumptions, vendor commitments, grant deliverables, and required supporting documentation. Track program milestones, expenses, and deliverables against applicable award requirements and maintain complete records for reporting, monitoring, audits, and closeout. Identify potential compliance, allowability, documentation, budget, or performance concerns and coordinate timely corrective action. Establish a reporting calendar and processes for collecting, validating, and presenting program, financial, service delivery, and outcome data. Produce concise dashboards and written updates covering milestones, participation, service delivery, spending, outcomes, risks, decisions, and corrective actions. Coordinate recurring government grant and program reporting across multiple contributors and data sources, resolve reporting gaps, and maintain documentation supporting reported results. Serve as the day-to-day operational contact for assigned Louisiana Department of Health program matters. Coordinate schools, community organizations, health plans, healthcare providers, contractors, vendors, and other partners involved in the awarded work. Prepare agendas, decisions, action items, and progress materials for executive, partner, advisory group, and steering committee meetings. Identify risks, issues, delays, and barriers; drive mitigation and resolution; and escalate material matters requiring executive or state level action. Coordinate applicable state and federal monitoring, audit, site visit, and reporting activities and maintain documentation supporting award compliance and program performance. Perform other related responsibilities consistent with the approved award scope and organizational needs.

Required Qualifications:

Bachelor's degree in public health, healthcare administration, business, public administration, nursing, or a related field, or a comparable combination of education and relevant experience. At least three years of formal project or program management experience, with direct responsibility for integrated implementation plans, schedules, budgets, risks, dependencies, deliverables, and executive reporting. At least two years of direct government grant-management experience, including award administration, budgets, compliance documentation, performance reporting, monitoring, and closeout for government-funded programs or awards with annual funding of at least $500,000. Demonstrated experience coordinating recurring government grant or program reporting across multiple contributors or data sources, validating information, resolving reporting gaps, and maintaining documentation supporting reported results. A current project management certification or recognized equivalent credential. Proficiency in computer use, internet research, and office software required (word processing, spreadsheets, databases, and presentations). Strong organizational skills and the ability to manage multiple priorities, deadlines, contributors, and workstreams. Ability to travel regularly throughout Louisiana.

Preferred Qualifications:

Experience managing government awards with annual funding of $1 million or more is strongly preferred. Experience with federal or state administered federal grants. Healthcare, rural health, school-based care, Medicaid, population health, or value-based care experience. Experience managing multiple grants or interdependent workstreams. Vendor contracting, negotiation, and performance management experience. An advanced degree in a relevant field. PMP is preferred;

CAPM, PRINCE2

Practitioner, or an equivalent recognized credential may satisfy the minimum requirement when supported by substantial applied experience. Proficiency with formal project controls, including integrated schedules, critical path dependencies, RAID logs, change control, milestone tracking, and executive status reporting. Experience using Microsoft Project or an equivalent project management platform, Microsoft 365, and standard reporting dashboards.

Work Environment & Demands:

This is a full-time position. Regular travel within Louisiana and occasional evening meetings may be required. Duties may be performed remotely, in an office environment, and in school or community settings. Regular travel to schools and other program sites is required. Candidates must have reliable transportation and be able to meet any applicable driver's license and automobile insurance requirements. Physical requirements include the ability to lift up to 25 pounds occasionally (e.g., office materials or supplies). May involve standing, walking or sitting for extended periods while some tasks may require bending, stooping, or reaching.
