Community Services/Community Housing/Horizon View Point Health is a community behavioral health center utilizing a team of psychiatrists, nurses, licensed counselors and social workers, as well as other professionals to provide services to individuals who need treatment and support to cope with mental illness, substance abuse, and intellectual and developmental disabilities. View Point Health serves uninsured, underinsured, low-income Medicaid, Medicare, war veterans and some private insurance across multiple locations with a full continuum of behavioral health services and support. The Residential Site Supervisor guides, and/or instructs the work assignments of subordinate staff. Assists with daily living activities at the person's home, in a daytime non-residential facility or a residential facility. Observes and monitors clients' behavior and charts observations and incidents. Provides training of basic personal and social skills, simple academics, or work skills to consumers/students in a residence, group home, workshop, or supported employment environment. May assist with meal preparation, ensuring or encouraging client group activity, providing transportation, and/or limited case management.
Duties & Responsibilities:
Identify, secure, and maintain suitable housing options for clients. Initiate contact with potential clients to schedule and conduct eligibility interviews, ensuring clear communication regarding the purpose and requirements of the interview process. Provide crisis intervention and emotional support to clients experiencing acute distress or crisis situations. Ensure accurate and timely documentation of service authorizations, care plans, progress notes, and incident reports. Manage vendor relationships as needed to maintain units, ensuring timely repairs and upkeep. Ensure site compliance with health, safety, and stakeholder regulations. Coordinate team schedules and assignments to ensure adequate coverage and efficient operation of the crisis respite apartments. Train staff on the core principles of the residential services program, including positive consumer interactions, respect for consumer rights and dignity, and the use of normalization principles. Provide leadership and support to staff as they work on goals and objectives identified in consumer treatment plans. Counsel subordinate staff individually on job performance, monitor compliance with agency and program procedures, and formally evaluate performance using prescribed methods and timelines. Participate in the recruitment, hiring, orientation, commendation, promotion, and disciplinary actions of subordinate staff. Reviews time records submitted by non-exempt subordinates for accuracy and completeness. Stays up to date on training as required by VPH and all other regulatory authorities and ensures the same for subordinate staff. Other related duties as assigned.
Minimum Qualifications:
High School diploma/GED and three (3) years of job-related experience providing social or direct care services to groups of special needs individuals in a human service setting, which includes one (1) year experience as a lead worker, team lead, or supervisory role; or One (1) year of experience at the lower-level Client Support Worker 3 (SSS012) or equivalent position.
Preferred Qualifications:
Experience working with clients with mental health and substance abuse diagnosis in a residential setting. Flexibility in managing daily tasks is of utmost importance. Certified Nursing Assistant certification (CNA), Direct Support Professional (DSP), Direct Care Professional (DCP) or Patient Care Technician (PCT) preferred.
Requirements/Competencies:
Must pass a pre-employment drug screen and subject to random drug screens. Must have a valid Driver's License and a clean Motor Vehicle Record in accordance with company policy. Must pass a criminal background check (including fingerprinting). Requires TB Evaluation at pre-employment and annually. Must be able to lift 20 pounds. Requires long periods of sitting/standing. Must have the following technical skills - Word, Excel and Electronic Health Record (Care Logic). Must be willing to transport residents in the company van. Must demonstrate the ability to maintain professional boundaries with clients in a residential treatment setting. Must be able to lift and carry groceries and supplies and move furniture for cleaning. Must be able to assist disabled individuals on/off vehicle and secure seatbelts. Must be able to use a gait belt and mechanical lift to lift and position individuals, as needed.
