Job Description
Pleasant Vine Home Care is looking for a Program Specialist to oversee the delivery of residential services for people with intellectual and developmental disabilities across several homes within a given area. The primary responsibility of this position is to provide supervision and oversee the implementation of all program goals, outcomes, and corporate policies and procedures. The Program Specialist is directly responsible for overseeing the implementation of consumers' Individual Support Plans (ISP) and consumer records, developing consumer plans/outcomes, and coordinating consumer services. The Program Specialist position is both a salaried and supervisory position that reports to the program director. Additionally, this position may work various shifts to provide appropriate supervision, including weekends, as necessary. Since this position is supervisory and salaried, there may be times that require this position to work more than 40 hours without further compensation. This position will also serve in the on-call rotation. Key responsibilities of the Program Specialist include : Must be fluent in the Individual Support Plans (ISPs) of the consumers assigned and provide individual and group training as necessary. Provides supervision to the direct care staff and consumers. Completes all regulatory paperwork, including monthly progress notes, quarterly reviews, residential assessments Must be well versed in PA Chapters 6400 or 3800 regulations. Oversees consumer progress and adjusted goals/outcomes as necessary. Serves as a consumer advocate. Must demonstrate excellent verbal and written communication skills. Must always exhibit a professional attitude. Achieve independence, growth, and happiness through high-quality, creative services for the people in the home. Adhere to all state regulations and enforce all agency policies and procedures.
Job Requirements :
Must possess strong knowledge of PA Chapter 6400 regulations. Minimum two years of experience supporting individuals with intellectual and developmental disabilities. Minimum one year of supervisory experience. Must be willing to undergo all required clearances Must have a valid driver's license. The Program Specialist must possess one of the following : A master's degree AND 1 year of experience working directly with persons with IDD. A bachelor's degree AND 2 years of work experience working directly with persons with IDD. An associate's degree or 60 credit hours from an accredited college AND 4 years of work experience working directly with persons with IDD. Essential Responsibilities of aProgram Specialist :
Supervise all aspects of programs to ensure individuals' health, safety, welfare, and personal development. Oversee service and planning for individuals. Ensuring that the outcomes in the individual support plans are implemented. Oversee the development of the home's employees. Ensure that regular meetings are held relating to the care of the individuals, including, but not limited to, monthly employee meetings at the home and treatment team meetings. Represent Pleasant Vine Home Care before the community, relatives of clients, and other human service agencies.Competencies :
