The Community Resource Coordinator (CRC) acts as a liaison with local community organizations to provide support to licensed personal care homes serving low-income populations.Minimum Qualifications:
One (1) year of professional experience in developing, implementing or evaluating human services, sociology, public health care insurance programs; and a bachelor's degree in human services or education Field OR Any equivalent combination of experience and training that includes at least one year of professional experience in developing, implementing or evaluating human services, education, sociology, public health insurance programs. Knowledge of available resources for low-income residents. Proficient Computer skills to include, but not limited to, Microsoft Office Suite. The ability to work with minimal supervision and independence. General research skills are required as needed. Problem resolution and critical thinking skills required. Experience in communication and relationship building. Professional communications skills, writing complex technical analysis and interpretive skills required.Job Duties and Responsibilities:
Serves as a liaison between licensed personal care homes and local agencies, professional boards, task force groups, churches and non-profit organizations. Collaborate with other agencies to develop and coordinate resources to establish effective working relationships for the licensed homes; receive and makes appropriate referrals with the licensed homes; monitors and analyzes the effectiveness of service resource delivery within department; maintains a database of resource providers and services; prepares reports as needed. Conduct on-site assessments of need within licensed personal care facilities to identify most appropriate resources that support low-income residents and provide technical assistance to the licensed personal care facilities to access available resources to residents and facilities such as applying for available funding, utilizing food banks, applying for eligible benefits, such as SSI, personal care home supplements, waiver programs, etc. Interviews residents, administrators and business managers of the homes to develop liaison relationships with other agencies. Meet with SSI home quarterly to assist the home with local social service agencies, church groups and other non-profit organizations. Train with Licensing Representatives on site to understand the essential functions of licensing process. Complete COPA, professional development and licensing trainings as assigned. Engage in problem-solving activities with licensed personal care facilities to identify solutions to emerging situations affecting residents. Attend regional Risk Management meetings to identify areas of concern, share resources and advocate for additional shared resources. Provides presentations and training to licensed facilities, Dept of Human Services staff and local community organizations such as county MH/ID programs, Area Agencies for Aging, local Ombudsmen, etc. Documents are written in clear and concise way. Provides a written report on weekly activities to the CRC Team Lead and Regional Director. Coordinates with regional directors and participates in staff meetings to identify facilities that may be at risk. Other related duties as required.Essential Requirement:
This position will be in the Department of Human Service's OLTL Bureau of Human Services Licensing. This position is a teleworking position. You must have a securely configured internet connection. The individual must be highly motivated to work independently and self-motivated to complete assigned tasks. The position provides support to the licensed homes and regional department as needed.Travel:
Regular travel within the region to attend meetings, liaison with resources and conduct on-site visits. The CRC needs a dependable vehicle and a flexible daily schedule. AL_TSC About Our Company:
