Job Description
PRIMARY RESPONSIBILITIES 1.
Promote the agency philosophy to ensure high levels of individual care through community integration, individual choice, and development of individuals' competencies, while treating individuals with respect and dignity. 2. Develop, review, and supervise the implementation of all residential programming for assigned individuals, including ISP program plans and program activities. Ensure that these programs promote individual growth. 3. Maintain and manage the electronic client records system to ensure proper and timely documentation, reporting, communication, training, and attendance. 4. Coordinate and complete assessments. 5. Participate in interdisciplinary team meetings. 6. Provide direct care as required or needed to ensure the safety and well-being of assigned individuals. This may include providing shift coverage, crisis intervention, transportation, and attendance at medical appointments. 7. Ensure the medical, emotional, and social needs of individuals are met. 8. Follow up on medical recommendations as required by applicable regulations. 9. Ensure accurate and efficient management of all financial records and accounts. 10. Ensure a safe, clean, comfortable, and therapeutic living environment through management of household responsibilities, including employee scheduling, home and vehicle maintenance, grocery shopping, menu planning, home improvements, and house cleaning. 11. Conduct MAR reviews and maintain training as a medication supervisor. 12. Review and approve timesheets, leave requests, travel reimbursements, and related documentation. 13. Supervise assigned staff, ensuring adherence to agency philosophy, policies, professional standards, and applicable federal, state, and county regulations. 14. Supervise the development of employees' skills and knowledge through orientation, training, and evaluation. Responsibilities include performance evaluations, staff meetings, and staff discipline/counseling. 15. Develop and maintain all initial and ongoing training related to electronic client records and ensure proper and timely documentation by staff. 16. Monitor data and progress on individuals' training programs, including observation of staff during implementation. 17. Maintain current information and required materials, including emergency packets, individual records, face sheets, and related documentation. 18. Serve as a Mandated Reporter. 19. Position requires some on-call, evening, or weekend work. 20. Perform other duties as assigned or needed.PROGRAMMATIC RESPONSIBILITIES 21.
Act as an appropriate role model. 22. Perform ongoing assessments of the service needs of individuals. 23. Counsel individuals and teach them how to define and solve problems by facilitating self-expression and other means of communication. 24. Encourage integration of individuals into the community. 25. Teach individuals independent living skills, including, but not limited to, money management, food purchasing and preparation, personal care, community mobility and resource awareness, day program/job responsibilities, use of leisure time, and interpersonal skills. 26. Provide transportation, when necessary, to work sites, appointments, and activities. 27. Advocate for human rights and the right to the least restrictive environment. 28. Provide assistance and input into the creation of the Individual Service Plan (ISP) by completing the appropriate feedback form. 29. Compile monthly documentation/data sheets and submit them to the Program Manager. 30. Prepare required paperwork, including medication charts, waiver schedules, daily log sheets, and other documentation for the following month prior to the end of the current month. 31. Prepare and submit quarterly reports documenting individual progress. 32. Assist with and provide input into the preparation of Waiver Plans. 33. Assist with writing checks for individuals' accounts to pay bills. 34. Perform other duties as required.HCBS RIGHTS AND RESPONSIBILITIES

Ensure individuals are integrated into and supported in accessing the greater community. 36. Ensure an individual's rights to privacy, dignity, respect, and freedom from coercion and restraint. 37. Optimize, without regimenting, individual initiative, autonomy, and independence in making life choices, including daily activities, physical environment, and with whom to interact. 38. Facilitate individual choice regarding services and supports and who provides them.QUALIFICATIONS
- High School Diploma or equivalent.
- Experience working with adults with intellectual disabilities.
- Competency in the area of skills training.
- Ability to assess and provide training based on individuals' needs.
- Familiarity with household operations.
PHYSICAL QUALIFICATIONS 39.
Free of TB communicable form. 40. Physically able to drive a vehicle and eligible for a driver's license. 41. Able to complete household chores. 42. Able to climb stairs and physically maneuver around the home and community. 43. Able to assist individuals with evaluation. 44. Insurable underINSIGHT'S
