Job Description
Job Purpose We are seeking a highly organized and detail-oriented Scheduling Coordinator to join our team in the Program of All-Inclusive Care for the Elderly (PACE). The Scheduling Coordinator will play a crucial role in managing and coordinating appointments for participants of the PACE program, ensuring they receive comprehensive and timely care services. This position requires strong communication skills, multitasking ability, and a commitment to providing excellent customer service to elderly individuals. Duties and Responsibilities Schedule and coordinate medical, therapeutic, and other health-related appointments for PACE participants in collaboration with healthcare providers and the interdisciplinary care team. Maintain accurate and up-to-date participant schedules, ensuring all appointments are scheduled in accordance with participants' needs and care plans. Communicate effectively with participants, families, and healthcare providers regarding appointment availability, changes, and cancellations. Monitor and track appointments to ensure all services are provided timely and efficiently, following up as necessary. Assist in managing transportation arrangements for participants, ensuring they have access to transportation for their scheduled appointments. Handle the rescheduling of appointments when needed, ensuring minimal disruption to participant care. Maintain detailed records of appointments, cancellations, and rescheduling within the program's scheduling system. Collaborate with interdisciplinary teams to ensure that care plans align with scheduled appointments and services. Ensure that all appointment scheduling complies with program policies and regulatory requirements. Assist with reporting and data entry as required, providing updates on appointment status and trends. Act as a liaison between participants, families, providers, and internal teams to resolve any scheduling-related concerns or issues. Other duties as assigned.
